Shockwave Therapy (also known as Acoustic Compression Therapy or Extracoporeal Shockwave Therapy) actually involves the use of sound waves or acoustic waves. These sound waves meet at a focal point in the target tissue (muscle, joint, tendon, etc.) and produce an intense, extremely short duration pressure pulse that delivers therapeutic massage in areas difficult to reach by other methods. What happens to the tissues treated with this therapy?

While the exact mechanism is not yet known, ESWT commonly leads to improved circulation due to blood vessel dilation in and around the injured area. Growth of new blood vessels has also been recorded.

Significant pain relief is almost immediately evident, although slight swelling and sensitivity may be noticed for a few days. ESWT also has a positive effect on the concentration of transforming growth factor beta 1, which stimulates cell activity. In addition, ESWT influences bone remodeling by thickening the outer layers and strengthening the cell network underlying joint cartilage.

ESWT has been shown to stimulate and accelerate the healing process. This is achieved by having an immediate analgesic (pain killing) effect and reducing the inflammation in the affected area.

Most treatment protocols to promote healing of acute injuries involve 1000-2500 impulses, delivered at the target area once every 2-3 three weeks for 3 treatments. The type and location of the injury determines the energy required, the depth of focus, and the number of pulses the tissue requires. The horses are usually restricted to stable rest and controlled exercise between treatments. The horses are then re-evaluated. Post treatment exercise levels are guided by the nature of the original injury and the improvement seen with treatment.
